**Participant should fill up this form and send to the coordinator in-charge. **Full payment should be made before the start date of the training as the course materials will be shared in advance. **Payment is not refundable. **In case of changes in location, time and date, we will inform you in advance. Participants has the right to continue on the said course, attend on the next training course or refund the 75% of the actual payment. **Number of confirm participant must attend on the said training; cancellation must be 4 working days prior to the training date, Laeq Food Safety Riyadh has the right to charge you depending on the confirm number of participants.**
